The Thunderbird Partnership Foundation provides the Native Wellness AssessmentTM(NWATM) to communities and organizations that provide Indigenous culturally-based programs and services to measure the impact of culture on wellness. To support the mandate of Thunderbird Partnership Foundation to advocate for First Nations culturally-based addictions and mental health services, Thunderbird will produce nationally aggregated reports based on personal information data entered into the NWATM. Personal information used, however, will not be identifiable to any individual.

When you click on the terms of use checkbox below, you agree and consent to Thunderbird Partnership Foundation’s intended use of personal information for the nationally aggregated data reports.

The Thunderbird Partnership Foundation, its officials, employees, agents, representatives, successors and/or assigns shall not be responsible for errors or omissions in the information or liable to you and/or any third party for any damages of any kind that you or a third party may suffer, howsoever caused, arising out of or relating to the use of the NWATM or Thunderbird’s reports including, but not limited to, any lost profits, lost opportunities, special, incidental, indirect, consequential or punitive damages, even if Thunderbird is advised of such possibility.

For existing users of the NWATM, accessing the assessment on or after June 19, 2017 implies your consent to Thunderbird Partnership Foundation’s intended use of personal information for the nationally aggregated data reports.
