This toolkit addresses renewal issues relevant to "Element 2: Early Identification, Brief Intervention, and Aftercare" identified in Honouring Our Strengths: A Renewed Framework to Address Substance Use Issues Among First Nations People in Canada.1 NNADAP workers and regional NNADAP needs assessments have clearly stated the need for a common set of Intake, Referral, Discharge, and Aftercare Planning processes and templates to ensure clients receive appropriate, streamlined care when and where needed along their recovery journey.
