Thunderbird Partnership Foundation (TPF) provides the Native Wellness Assessment (NWA) to organizations that provide Indigenous culturally-based programs and services to measure the impact of culture on wellness. To support the mandate of TPF and advocate for Inuit and First Nations culturally-based addictions services, TPF will produce nationally aggregated reports based on personal information data input into the NWA. Personal information used however, will not be identifiable to any individual.
When you agree to the terms of use box below, you agree and consent to TPF’s intended use of personal information for the nationally aggregated data reports.
TPF, its officials, employees, agents, representatives, successors and/or assigns shall not be responsible for errors or omissions in the information or liable to you and/or any third party for any damages of any kind that you or a third party may suffer, howsoever caused, arising out of or relating to the use of the NWA or TPF’s reports including, but not limited to, any lost profits, lost opportunities, special, incidental, indirect, consequential or punitive damages, even if TPF is advised of such possibility.
If you find any errors or omissions in your information, please report them to nwa@thunderbirdpf.org.
